About Dovetail Health - Dovetail Health is focused on providing care transition support, mostly for Medicare and Medicare Advantage patients, following a discharge from a hospital or skilled nursing facility. In order to help avoid preventable admissions, Dovetail has expanded on models of transitional care developed by Eric Coleman, Mary Naylor and others. Dovetail has developed a Pharmacist led program that includes in-home visits with patients and their families within 24-48 hours of discharge. Dovetail's infrastructure includes risk stratification, extensive training, perfected clinical work flows and protocols, and an internally developed Transition Management application tool. Dovetail and its partners collaborate to seamlessly integrate a care transitions program into the existing continuum of care with the goal of reducing readmission rates and ultimately the bottom line.
