Division of Financial Responsibility (DOFR)
The information contained in the Integrated Healthcare Association's ("IHA") Division of Financial Responsibility (DOFR) tool is copyrighted by the American Medical Association ("AMA"). This includes items such as CPT codes.

END USER POINT and CLICK AGREEMENT
You are authorized to use the CPT contained in the DOFR only as provided for herein. You agree to take all necessary steps to insure that your employees and agents, or anyone else with authorized access to the DOFR, abide by the terms of this agreement. Others that require use of the DOFR will independently register and accept the terms of the End User Agreement to access the DOFR.
IF YOU ARE ACTING ON BEHALF OF AN ORGANIZATION, YOU REPRESENT THAT YOU ARE AUTHORIZED TO ACT ON BEHALF OF SUCH ORGANIZATION AND THAT YOUR ACCEPTANCE OF THE TERMS OF THIS AGREEMENT CREATES A LEGALLY ENFORCEABLE OBLIGATION OF THE ORGANIZATION. AS USED HEREIN, "YOU" AND "YOUR" REFER TO YOU AND ANY ORGANIZATION ON BEHALF OF WHICH YOU ARE ACTING.
CPT is copyrighted by the AMA and is a registered trademark of the AMA. You acknowledge that the AMA holds all copyright, trademark, and other rights in CPT. CPT included in the DOFR is licensed from the AMA, pursuant to a nontransferable, nonexclusive license, for the sole purpose of internal use by you within the United States, only. The provision of updated CPT in the DOFR is dependent on the continuing contractual relationship between IHA and the AMA.
You are authorized to make copies of the CPT codes for back up or archival purposes only. All notices of proprietary rights, including trademark and copyright notices, must appeal on all back up or archival copies made. Any use not authorized herein is prohibited, including by way of illustration and not by way of limitation, making copies of CPT for resale and/or license, transferring copies of CPT to any party not bound by this agreement, creating any modified or derivative work of CPT, or making any commercial use of CPT. You are prohibited from publishing, distributing via the Internet or other public computer based information system, creating derivative works (including translating), transferring, selling, leasing, licensing or otherwise making available to any unauthorized party CPT, or a copy or portion of CPT. License to use CPT for any use not authorized herein must be obtained through the AMA, CPT Intellectual Property Services, 515 N. State Street, Chicago, IL 60654. Applications are available at the AMA Web site, http://www.ama-assn.org/go/cpt .
You, your employees and agents acknowledge and agree that you reside or work in California.
In the event any provision of this End User Agreement is determined to violate any law or is unenforceable the remainder of the End User Agreement will remain in full force and effect.

Applicable FARS/ DFARS Restrictions Apply to Government Use. This product includes CPT which is commercial technical data and/or computer data bases and/or commercial computer software and/or commercial computer software documentation, as applicable, which was developed exclusively at private expense by the American Medical Association, 515 North State Street, Chicago, Illinois, 60654. U.S. government rights to use, modify, reproduce, release, perform, display, or disclose these technical data and/or computer data bases and/or computer software and/or computer software documentation are subject to the limited rights restrictions of DFARS 252.227-7015(b)(2) (November 1995) and/or subject to the restrictions of DFARS 227.7202-1(a) (June 1995) and DFARS 227.7202-3(a) (June 1995), as applicable, for U.S. Department of Defense procurements and the limited rights restrictions of FAR 52.227-14 (December 2007) and/or subject to the restricted rights provisions of FAR 52.227-14 (December 2007) and FAR 52.227-19 (December 2007), as applicable, and any applicable agency FAR Supplements, for non-Department of Defense Federal procurements.

End Users do not act for or on behalf of the Integrated Healthcare Association (IHA). IHA DISCLAIMS RESPONSIBILITY FOR ANY LIABILITY ATTRIBUTABLE TO END USER USE OF THE CPT. THE IHA WILL NOT BE LIABLE FOR ANY CLAIMS ATTRIBUTABLE TO ANY ERRORS, OMISSIONS, OR OTHER INACCURACIES IN THE INFORMATION OR MATERIAL CONTAINED ON THIS PAGE.
The DOFR is provided "as is" without warranty of any kind, either expressed or implied, including but not limited to, the implied warranties of merchantability and fitness for a particular purpose. You agree to indemnify the IHA for any and all claims arising from your unauthorized use of CPT contrary to the terms of this Agreement.
IHA does not assign or recommend use of fee schedules, relative values, conversion factors, related components, contracting and/or designation of risk. The IHA does not directly or indirectly practice medicine or dispense medical services.
This license will terminate upon notice to you if you violate the terms of this license. The AMA is a third party beneficiary to this license.
