Supporting Practice Transformation in California
The Pacific Business Group on Health and its California Quality Collaborative, in partnership with IHA and the Center for Care Innovations, will receive up to $18.4 million from the Centers for Medicare & Medicaid Services (CMS) to develop a Practice Transformation Network (PTN), one of 29 nationally. This PTN will provide technical assistance to help 16 provider organizations with 4,800 clinicians to expand their quality improvement capacity, learn from one another, and achieve common goals of improved care, better health, and reduced cost.
IHA will support this PTN by leading data collection and reporting by expanding its P4P web reporting portal, increasing the frequency of data collection and reporting to quarterly, and expanding data to individual clinicians within the physician organizations. The project also creates the opportunity for Medi-Cal (California’s Medicaid) plans to learn from peers and experts about best practices related to data collection and reporting and related performance measurement topics.
